This online reporting tool is intended for UT Health RGV Employees to submit occurrence reports, near misses,and complaints.

Non-employees are encouraged to report concerns directly to the site manager or supervisor to allow an opportunity for immediate resolution.

If resolution is not possible, further assistance may be requested through the facility’s administrative office.
